Note that to obtain a paper copy of your medical file, you must submit a written, dated, and signed request, along with proof of identity (a copy of your identity card). A fee will be charged for paper copies of all or part of your medical file to cover printing and administrative costs, including sending the documents via registered mail, up to a maximum of €50. Due to legal requirements, medical files cannot be sent by email or other digital methods.
